The Social Worker will provide intensive, strength-based preventive services to families whose children are at risk of foster placement. Social Worker Reports To: Social Work Supervisor, Neighborhood Family Empowerment Center Location: 2471 Morris Ave. Bronx, NY 10468 Hours: 35 Hours/Week, Exempt Social Worker Major Duties: The Social Worker conducts assessments of children and families referred by the Administration for Children s Services (ACS), schools, other agencies and community-based organizations, or those who come in voluntarily seeking services. Provide individual, family and group counseling to children and families based on a strength-based model. Maintain a regular caseload in accordance with the program s policies and procedures. Provide short-term crisis intervention and concrete services including advocacy, outreach and referral for designated caseload, as well as emergencies. Conduct home visits and escort program participants to appointments as needed. Plan, coordinate and implement groups and social activities for children, parents and families. Schedule, facilitate and/or participate in required conferences with families, community partners and ACS using the Family Team Conferencing (FTC) Model when indicated. Prepare appropriate statistical reports (PROMIS), FASP recording and complete casework and group work summaries as needed. Attend and participate in program staff meetings, as well as related agency training as appropriate. Implement and integrate the principles and procedures of Solution Based Casework: assessment, family and individual level outcomes, action plans and documentation and celebration. Actively participate in case consultation, continued SBC training and the SBC certification process. Perform other duties as assigned. Social Worker Qualifications: MSW preferred, or MA in related field Bilingual (English/Spanish) preferred. Must be able to work two evenings per week until 7:00 p.m. Excellent organizational, written, and communication skills. Must be proficient in Microsoft Office. Must have a commitment to work from a strength based and/or youth development perspective.
